TMS320 chip failure

I have noticed a particular mode of failure on TMS 320 that I have not seen on other microprocessors:  After months of functioning properly, the chip begins to overheat even when it is not driving anything.  This may be accompanied by the failure of one or two pin outputs.  I have seen this failure on several designs including eval boards.

Has anyone else seen this mode of failure?  Were you able to correct the problem?
