Table entry two needs to be corrected or you could tell me why it is 4096 instead of 4095.

    Hi Jan,

    It is easier to understand if you start with the "12-Bit Digital-to-Analog Formulas".  There are 4096 distinct levels (not 4095) because 0 is the 1st level, 1 is the 2nd, .. up until 4095 which is the 4096th level.  Because of this, the divider is 4096 and not 4095. 

    You can then rearrange to get the Analog-to-Digital formula you referenced.
