Hi Jan,
It is easier to understand if you start with the "12-Bit Digital-to-Analog Formulas". There are 4096 distinct levels (not 4095) because 0 is the 1st level, 1 is the 2nd, .. up until 4095 which is the 4096th level. Because of this, the divider is 4096 and not 4095.
You can then rearrange to get the Analog-to-Digital formula you referenced.
