The load is a DC 20V brush motor, and the maximum current is about 0.8A. The result of short circuit R18 is the same. The average voltage of R18 is less than 200mV, and the starting moment of PWM cycle is more than 200mV
The following is the duty cycle 90%, frequency setting 20kHz, R18 voltage waveform and output current

How many devices were tested? Even peak voltages exceeding 200mV can damage the device. Can you test a brand new device with R18 shorted and see if there are issues?
It would also be good to get plots of the input (PH/EN) and outputs (OUT1/OUT2) to see if the device is behaving as expected.
