How is it that the DRV2667 when powered from 3V3 or 5V can output > 100V to drive an actuator?
How is this amplification beyond the supply rail possible?

Yes with a 3.3V or 5V supply the DRV2667 is able to deliver 105V peak.
This is possible because the DRV2667(as well as DRV2665,2700,8662) have integrated boost converters (https://learnabout-electronics.org/PSU/psu32.php) . A boost converter is a DC to DC converter that can boost a low voltage supply into a higher voltage and lower current supply.
We use boost converters in many parts where the high voltage rail needed for an amplifier is not present in customers (typically battery powered) system. So we generate our own!
