DRV8323: VGHx goes higher than spec

Dear TI experts,

I owe you much, given this is like my 10th post, yet I'm here looking for your help again!

I'm developing my own custom board using this chip, and I've been having some burning issues, shorted half-bridges and drivers. Ringing and thermals were the primary suspects, but yesterday I found this while measuring the GHx.

Yellow is GH->SH and blue is GL->SL

As you can see the GH goes to 13.5V, which is out of spec for this driver:
 (Vmax = 13.5V)

And this is measuring GH, GL and VM->VCP (purple)

This is a GH measurement, but this time in the evaluation board, which stays on spec. (Vmax = 12V)


I'm assuming the broken boards I'm getting are because the gate driver is burning.

I've tried to reduce the IDRIVE setting, but I'm getting the same waveforms.

How can I fix this?
