in my design,
the input signal(sig_in) is 0 to 4.096V DC coupeld.
expected to output signal of 0 to 100V
output current is below 4 mA.
attached electrical scheme.
The most important point to review is critical electrical connection mistake that requires re-layout.

I am replicating your set up with our EVM and your current configuration would give an output as below:
If you change the gain to 28.8 dB, (changing the gain0 pin to ground) with the same input signal you get the following output which you can see has a little bit of clipping:
Still at 28.8 dB but changing IN - to 2 V, you get the following output:
I recommend doing the last of the 3 options as this is a typical set up for set reference single ended DC coupled input (See section 5.4 in the DRV2700 EVM user guide for more info) though I wanted to share a few scenarios incase a different output was wanted.
Also, your output LPF has a much higher cutoff frequency than your input LPF. I recommend lowering the cutoff of your output filter close to that of the input filter. This is not necessary, but will reduce the noise in the system.
Otherwise your schematic looks good to me.
