Thanks for your patience.  

It appears this may be an overcurrent fault as we see the driver temperature sharply rise during fault and operation. It may be that the overcurrent trip point is too high and the driver fails to shutdown before damage.

Does this fault shutdown the external mosfet operation when flagged? 

I also notice the IDRIVE setting is set very high (450/900mA). Can we slow the gate current down to 50/100mA (IDRIVE tied to GND) and reevaluate?

The MOSFET was not driven successfully, and the temperature of the pre-driver rose sharply and was damaged.

The customer has tried lowering the ID and is indeed able to turn the MOSFET on and off. He has adjusted the gate resistor and ID configuration values and made a prototype. It will take time to verify whether this issue can be resolved.
