DRV8231A: Internally shorted driver - Catastrophic failure of case material

I have a handful of drivers that internally shorted with and without catastrophic failure of the case.
- All pins with the exception of VM and VREF are shorted to ground.
- VREF is ~500 ohms to ground and VM is 800k ohms to ground. (Dead chip removed from board)
- IPROPI is set to 2.8 amps, with the entire driver being implemented nearly identically to the reference circuit.
- Under any motor actuation that I can produce, I do not see any excessive heat (<160*F at the top of case)
- The motors being driven are rated at 2.5 amps, drawing significantly lower than that after inrush.
I have tried erratic actuations of the motor, including changing directions while still in motion.
I am looking for any information/experience on failure modes/troubleshooting as I have been unable to reproduce this issue.
