Are the GDIC pins correctly protected using the recommended RC circuits?
Is the MOSFET itself being short circuited? I recommend referencing the datasheet sections on the DESAT/SC/OC protections (Sections 6.3.5.7 & 6.3.5.8).
I also recommend looking into the datasheet section on two-level soft turn off for some guidance on how to use the 2STO feature to decrease the voltage overshoot in these kinds of scenarios (Section 6.3.2.7).
