DRV8302 Fault

Two co-workers and me have been testing a hub motor with an ESC based on the DRV8302 (Schematic), and it worked perfect for some time, but suddenly we have a Fault and the ESC stopped working. After some hardware debugging we concluded that the DRV8302 has an internal failure because it has a short circuit between BST_A and SH_A and both stuck at the same voltage of GVDD (approx. 10 V).
Therefore, we proceeded to remove the damaged DRV8302 and solder a new one, ensuring that the PowerPad was properly connected. After, we see the following diagnostics:
  • BUCK OK.
  • DVDD = 3.3 V.
  • PVDD  = 36 V.
  • GVDD = 11 V.
But FAULT Pin is active. What could be the problem? What other test could we carry out to find the problem?
