DRV10983 EVM Damaged after first power on

I am having an issue with the DRV10983 EVM.  On first power up I use the out-of-box quick setup guide to get the motor that was provided with the board running. Everything works fine I can get it running in both directions, read and set all parameters.  After testing the motor I power down and power back on and the DRV10983 gets damaged. VCC shorts to ground.  I have replaced the DRV10983 chip twice already with the same results.

Just wondering what you think is causing the chip to become damaged?

I have my power supply set to 24VDC, the leads from the power supply are about 2ft.  I have the power supply limited to a max current of about 800mA.
