TPL7407L is burned

Hi Team,

The customer is using TPL7407L. But TPL7407L is burned in his board. Here are the schematic. The EN pin is connected to the Relay5 pin. Look the attachment.

Can you tell me why TPL7407L is burned?

    While waiting, can you find out:
    1) What is the part number of the relay
    2) How much current does the relay require when activated?
    3) How many relays are being activated simultaneously
    4) Is there a bulk capacitor on the +12 (COM) signal and what is the value
    5) What is the other side of the relay coil connected to?

    Just based on an initial look at the provided schematic, I do not see any issues that immediately stand out. 

    My initial guess is that the device is sinking too much current. Beyond what is recommended per channel or for the device as a whole.

    Please provide the information that Rick has requested, in addition to how often each of the relays are being activated.
