DRV8302: Unknown Fault

I am using the DRV8302 driver, it works fine normally but some times after quick changes of the PWM duty cycle the output gate drivers stop driving. The fault  the OCTW pin are both still high. I can clear the error by cycling the EN_GATE pin. I cannot find any situation in the datasheet that meets this criteria. All the faults that can be cleared by toggling the EN_GATE pin should set the ~FAULT or ~OCTW pin. 

Is there a rare situation where this is not the case?

    Can you provide more information? Is the PWM duty cycle being increased or decreased?

    What is the starting duty cycle and the target duty cycle when the gate drivers stop.

    Have you looked for small glitches on the EN_GATE signal? From the datasheet:

    An EN_GATE reset pulse (high → low → high) from 10 to 20 μs should not be applied to the EN_GATE pin. The DRV8301 has a transition area from the quick to full reset modes that can cause the device to become unresponsive to external inputs until a full power cycle. An RC filter can be added externally to the pin if reset pulses with this period are expected to occur on the EN_GATE pin.

    Have you monitored the nOCTW  and nFAULT pins during this time? Is it pulsing?
