DRV8312: search for DRV8312DDW damage reason

Part Number: DRV8312

Currently we use the DRV8312DDW device with power pad down in our application.

The devices worked well till now, but at the last time some devices were damaged.

According to  data sheet heading, this device is limited to 3.5A continuous current when

using the thermal pad on the bottom side of device.

We set the current limit to 4A, and we use CBC current limit mode. 

May be the limit of 4A was too much ?

Also I would like to ask if there is any problem/errata related to devices batch,

the damaged devices were signed as following:

    DRV8312

    62COZXTG4

    Can you provide a few details?

    How long have you used the DRV8312?
    How many failures have you seen?
    Out of how many devices?
    Are the failures only assiociated with the 62COZXTG4 code?
    What is the failure?

    Dear Rick ,

    Thank you for fast reply.
    Additional details are following :

    - We use DRV8312 about for 2 years.
    - Some time ago we performed minor change in microcontroller SW version that delivers PWM to driver.
    - Before the SW version change the motor driver worked well (device with code 62COZXTG4) .
    - After the SW version change, running the motor driver under motor load caused to irreversible damage, i.e. the reverse to previous SW version does not help ...
    - The problem occurred till now in two devices, we did not try to run the new SW with another drivers from the same batch yet.
    - The failure seemed as "lost of driver strength", i.e. the motor can't be able to deliver enough moment again the motor's load soon after the start up.
    - After replacement of the above devices with another ones (with code 38C34NTG4 ) - the design returned to work well again, but with old SW version.
    - The additional design with driver device signed 38C34NTG4 works well also with new version of SW.
