DRV8701: DRV8701E nFault always low , and MOSFET disabled

Hi,

I'm using DRV8701E to to driver 12V BDC motor, when set forward mode via EN/PH,  the nFault pin is low at the time.

but the VCP and AVDD, DVDD are generated well.  after check datasheet, it should not UVLO on VM(+12V) and VCP (21.6V);

we can AVDD=4.8V, DVDD=3.3V already.   we set  IDRIVE into 25/50mA mode.

but the motor has doen't work. 

Vds =12V all the time.  don't know how to let motor work.  i paste my circuit here.

Any ideas?
