DRV8432: DRV8432 burn GND-C/D and OUT-D when 24V power up

Recently we have a product that use DRV8432. But there are 4 pieces out of 200 had burn the OUT-D and GND-C/D, OUT-B and GND-A/B when 24V powered up.  Can anybody tell what is the potential reason? thanks a lot.

    Can you provide a few details?

    Please note the recommended min OC_ADJ resistor in Cycle by Cycle mode is 24kOhms. How much current do you need?

    Are you using a heat sink? If use, what is the size of the heat sink?

    The image of the device shows many of the leads appear to be detached from the PCB. Please inspect the working devices for solder connections.

    What is the PWM frequency?

    What is the lot trace code of the devices (a photo of the top of the device showing all markings will help)?

    Please check the picture showing the burned device. The trace code is 8BZNX74G4.

    We have used this chip for over 1 year, this is the first time the device burned. The burn happened when power was up, the PWM was not running. The heat sink is fine on other devices. 

    Maybe this batch has some problem. Hope you can help to find it.
