DRV8350: DRV8350: Driver failure

DRV8350RH driver fails after IDRIVE bias resistors are not install. We bias the IDRIVE pin with no pull up resistor plus 75K pull down or 18K pull down resistors and it works fine. However, we are seeing many fault signals. The fault signal increased with the load current. With no pull up and no pull down resistors, the driver failed. VGLS went down to 3.5V instead of 12V. The driver no longer functions. We lost 3 DRV8350RH chips in the same condition. The TDRIVE (VDS) bias function correctly with all bias combinations. 

Any suggestions why the fault signal keep occurring? Any reasons why the driver burns when there are no bias resistors to IDRIVE pin?
