Can you provide scope captures of the inputs and outputs when the device is:
unloaded (not connected to the motor)
loaded (connected to the motor). When loaded, providing the current through the motor will also be beneficial.

Xin/Xph or Xin2/Xen should be connected to a control circuit. This can be a GPIO, PWM input, or a static voltage. Please refer to Table 3 and Table 4 for how to connect the inputs.
The outputs should follow the Table 3 and Table 4. If not, there could be a fault condition causing the outputs to be disabled.
Having the scope captures will help determine if the outputs are properly following the inputs.

The inputs are all digital inputs.
The outputs are 0 to VM voltage transitions based on the input settings in Table 3 and Table 4.
Please provide a picture of your connections to the EVM and scope captures of the input and output voltages.
