DRV8842: The output of DRV8842 does not match the datasheet

Hi Team,

The customer recently used the H-bridge to drive DRV8842, the schematic diagram is as follows:

After using DC12V power supply, FPGA is used to generate low-frequency PWM with amplitude of 3.3V.

There is a glitch at this time. When IN1 is a PWM waveform and IN2 is grounded, the output waveform does not match the logic in the data manual:

At this time, the output waveform OUT1 is lower than OUT2, and the output voltage is only about 5V. Exchange IN1, IN2 state, that is, IN1 is grounded, IN2 is a PWM square wave, and the output waveform is unchanged. Customer would like to know what is the reason?

    We have a few follow up questions: 

    1. What is the voltage on VM? A scope capture would be helpful to see if the voltage is collapsing. 
    2. What is the value of the sense resistor? 
    3. What are the other inputs IN4 through IN0? 
    4. Is nFAULT asserting? A scope capture zoomed in to 10ms/div would be helpful.
