DRV8840: DRV8840PWPR random nFAULT high during softstart

I've encounter an random nFAULT high with the DRV8840PWPR motor driver during soft start which makes the motor stop. The Issue is only occurring in 10% of the DRV8840PWPR circuits from our boards, and replacing the circuit usually fix the the problem.

In the circuits where the problem occurs the fault is usually only in one direction (PHASE high), meaning that the other direction still works fine (PHASE low). The problem is also mainly occurring on the DRV8840 located to the left in the layout.

I would very much appreciate if someone wants to take a look at the design and see if there is anything wrong with it. Please find the schematic and layout for the DRV8840 part of the design in the images below. Both front and back copper pours are connected to GND.
