DRV8876: IC failure

Hi,

I am using DRV8876PWPR in my custom design for DC Motor.

Observed IC failure - pin short between pins 11 (VM) and 12 (VCP) by measuring the voltage at charge pump and found the supply voltage (VM) and charge pump (VCP) output are same.

Suggest reason for failure.

Did you notice this short in other units or is it isolated to this one failing unit?
