BQ2057C: Problem charging Li ion battery

Greetings! I`m using BQ2057CTSTR for a very first time. Here is my schematic -> 8875.BQ2057.pdf

I used figure 1 from the datasheet as a model. The problem is I burned the battery. I think the BQ2057 too. It upplied max voltage on the battery and now it is charged above the 4.2V value. I am using the 4.2V BQ2057 version as you can see from the marking. Any ideas what did I do wrong?

One more thing! I think there should be a diode preventing reverse voltage from the battery. Now when I connected the battery the STAT light turned on, even before I had applied the supply voltage. My supply voltage is 12V DC (sorry for the mistake in the schematic!) I`ve just noticed it!
