Having trouble with this chip TLV62568. Input is failing shorted to ground. has happened on 3 parts back from field. one failed while I was looking at the returned product. The 5 volt input was already being pulled down to 4.5 volt and the output was 4.5 volt also. Switch pin was switching ~3.3v to 4.5v (not to gnd), output was at 4.5 volt (not 3.3v) then a few minutes later the chip totally failed and the input shorted to ground. Replace the TLV62568 and all is well. any ideas?
