TPS54821: TPS54821: RT/CLK Left Floating

I have a design with the TPS4821 that had left the RT/CLK Pin Floating.  After installing a 47.5K Resistor to GND, I see the PH signal toggling @ around 1 MHz for 5 - 6 times and then switches to 500KHz.  Wondering if this is in Pulse Skipping Mode for Light Loads.  Is there a minimum Load Requirement?  The output voltage is 0.4V when designed for 1.0V.   Is it possible the device was damaged due to a floating RT/CLK pin?

Below is the schematic of the circuit.

Vin/PVin = 5.0V,

1V5 is up to 1.5V

EN is High at 5V. 

R90 is depopulated in order to isolate the supply from the Load Circuitry.
