TPS92692: Buck-Boost LED Driver Appears to be Faulting Every 6ms

One of my designs is using the TPS92692 configured as a Buck-Boost. The device will be driving 3 white LEDs in series at 1.3A with an input voltage requirement of 8-16V. I have followed the online calculator for the device but when I power up my circuit my LEDs come on dim. I have done some probing with the oscilloscope and I have identified that the fault pin goes to ground around every 6ms. At this point I have tried a lot of different things to try to keep the chip from faulting with no major success. Any help/ suggestion would be appreciated.
