Hi Ritchie,

a few comments:

Review the connection of the Sense resistor at CS and CSG. There is one wire where the full current needs to pass through which is to thin for my opinion.

I would also add here a few more vias to the GND plane.

Thanks for taking the time to review my board.  
Yes, after looking at the board again, the track width is too thin.
That being said, how does the track pad of the IC handle such a large current?
