LM4510: IC failing to regulate output after a few hours of use

We have a design using the LM4510 part to boost a single cell LiPo battery to 12V to power an OLED display. We have had 2 cases where the IC has worked normally, then after a few hours of intermittent use, the IC fails to regulate the 12V output. The schematic matches the typical application circuit from the datasheet, with the feedback resistors modified for 12V regulation.

All support components appear to be in spec and functioning as expected. Any idea on items to check to understand what has caused this issue?

    What is the load current? Could you please provide Vin, Vout, SW and inductor current when the chip fails to regulate 12V? Also, the temperature at this point is also important if possible, because maybe the chip temp is too high because of bad layout, etc. If the temp is very high, please also provide layout to review. Bad layout may cause the heat unable to dissipate.
