  1. TLV62569DBVR as a secondary power supply output 3.3V use, the primary power supply for TPS54260 output 5V voltage, power supply range of 9 -- 36V. 
  1. The TLV62569DBVR works as follows    
  2. All the problems of the equipment are TLV62569DBVR. The primary power supply is not damaged, and the power terminal is not damaged. The TLV62569DBVR can be used normally after replacement.  Chip damage generally starts two months after the equipment is put into use. 
  1. Up to now, 15 sets of equipment have been repaired, and the number of repairs is increasing with the increase of sales. 
  1. All the equipment has been checked before shipment without any problems, and any problems will be damaged after customers have used it for a period of time.  The customer usually uses a 12V/24V switching power supply in a cabinet. 
  1. After the problem occurred, we tested the power supply of the equipment for destructive surge and pulse group (5000V) and no damage occurred 

 Need help:

  1. Please help to analyze the cause of chip damage.
  2. Whether the circuit we are using can be optimized to avoid damage.

    Firstly, I have reviewed the schematic, output capacitance value looks out of the range of LC matrix to what is recommended in the datasheet.

    You have more than 100uF capacitance at the output whereas 10uF is the most recommended value in the datasheet with 2.2uH inductor for voltages greater than 1.8V at output.

    I have few questions to dig deep into the issue.

    1) Can you share which inductor (part number) you are using?

    2) Just to confirm that I have understood it right, damage is happening at customer's side?

    3) Is there any failure analysis performed by you already on damaged devices?

    4) Can you provide detail info about the end equipment?
