Hey Tommy,
The hiccup mode should happen if the output is shorted. However, what can happen is there can be peak charging in the auxiliary winding of the transformer. Depending on the transformer, a resistor or RC filter may be required to deal with this peak charging.
This is covered in the datasheet in section: 8.2.2.7 Switching Transformer Considerations and RVC
I would measure the voltage on the VCC pin during this time to determine if there is a spike.
Additionally, what about the IC is damaging? Using the PoE PD Debug Guidelines, can you determine where in the board the damage is occuring?
