TPS61022: Intermittent failure . Component smokes.

We are having an issue with the TPS61022 with intermittent failure of TPS61022 (U2) being charged by various USB style chargers. Do you think we need diode to pre-bias the voltage? Or is it layout or parts selection problem?

    Thanks for reaching out. Please pre-bias output before you power up the device. You can refer to datasheet 8.3 for reference. For layout. I recommend you strictly refer to datasheet layout recommendation. You have your output capacitor far away from VOUT pin. This is not good. The cross-connection on capacitor's GND lead is not recommended. This will rise the parasitic into VOUT loop.
