BQ25895: We have had issue with exploding chip (chunk out of body is 'blown off' - see attached picutre)

Its very strange - the design works perfectly in normal operation (and does on numerous other designs). Its only on attaching/detaching wires from Lab supply during testing.

However this one is a 12V design and this only occurs when we connect / disconnect the wires seperately from lab power supply. Also noticed when input wires were accidentally shorted together when disconnecting from lab power supply

It also happened on the BQ25890, so I thought it might have been the input wires shorting and OTG, but obviously the BQ25895 can't do that.

2. The lab supply wire might introduce electrical over-stress on the device. Please use as short as possible lab supply wires and "on/off" button on the lab supply instead of attaching/detaching wires physically.
