TPS2420 issue

I have a customer with the following issue:

I’m debugging a new board that I designed with the Texas Instruments part TPS2420RSAT. It’s not working right. Let me give you some info:

 

a.       Vin = 3.2 to 3.3Volts DC

b.      Vout = 0.5 V

 

If you take a look at the datasheet, pg 2, under Recommended Operating Conditions, the VIN/VOUT is set from 3V min to 20V max. See below a snap shot of my schematic and how I configured it. I based the values of R34, R26, R 19, and C20 from 20 under Typical Design Examples for fault current of 1 A. If you look at figure 25, the reference design has pins 14 and 15 fedback to the input through 10k resistors. Now on the pin description on pages 7 and 8, both PG- and FLT- are open drain outputs. I inquired directly to TI when I was designing this but didn’t get any good information. As you can see, I left them floating.

 

Would you please take a look at this and let me know if you can help me debug this problem?
