BQ29209: Question about avoiding failure mode after many cycles

I use this part to balance a 2S battery. After a very long time (several hundred total charge cycles of the battery) there have been 2 units that have failed.

I noticed that the battery became very unbalanced, and when I probed RCB above, I noticed that VC1 was discharging to ground in one case, and VC1 was discharging into VC1 in another case.

In the above block diagram it's fairly clear that the two mosfets on the left are the culprits here. After a certain amount of use they just fail short it seems.

Do you know of any way to avoid this failure mode?
