bq20z95 based battery disabled for 1 hour

The bq20z95 based battery pack, 4S3P, opens the Discharge FET possibly after a high Discharge event.  It doesn't close the circuit, however for a min of 1 hour.  Thus the battery appears dead to the user until the circuit recovers.  These are based on field accounts.  I haven't been able to duplicate it in the lab.  Any ideas what could cause this to happen.

    Which safety flag was set? What was the current that trigger the fault and how long did the high current present? What is the safety threshold setting (this can read from gg file).

    We need to know the step by step event that lead to the fault, and what action the customer did after the fault occur.

    I'm asking if anyone has seen the chipset take significantly longer to release than the 8 sec stated in the documentation.  Typically the customer removes the battery, thus the load drops down to 0mA  discharging outside the battery. By the time the battery returns to the lab, it is working properly again.

    Are the safety flags logged internally?
