TPS23754: PD can't output when PSE plug in(TPS23754)

Hi

We found our PD PCB sometimes can't power on.And this is not happened on all PCB.

I found that the issue was happened easily when VOUT of un-normal PCB power on "with loading"

Please see attached circuit.In our design,  Vout = 15V.

 And I caught some waveform in normal case and un-normal case of tha same PCB

Could you help to check design and waveform to clarify the issue. Thanks.
