In formula 26 in the SLUA303 document a 5V VDSmax is assumed. But in table 2 a VDSmax of 15V is stated. Is that a mistake or how is the 5V calculated?

The 15V listed in Table 2 is the rated VDS of the SR. The 5V used in equation 26 is the worst case VDS under the conditions stated around that equation. For self-driven SR, the VDS of one SR becomes the VGS of the other and the vice versa. The VDS (VGS) values are determined by VIN scaled by the transformer turns ratio.
