In the first link, they revised that app note again earlier this year. So, it is Rev G now: http://focus.ti.com/general/docs/litabsmultiplefilelist.tsp?literatureNumber=slma002g

TI can provide the requested thermal numbers, but it requires a TI employee to access the last link on that page and enter the required information. This can be done by your local TSR or your resource in the ACAT for those distributors in the US. I have done this for you for this request, so you don't need to do anything. We should have the analysis back within a week at most.
