our customer is using LM5009 since a while already in one of their product and recently encountering some issues.
Could you please have a quick look into below?

The component is LM5009, we have been using this component for more than 10yrs in our product and recently we are having report that the component is having a failure mode of pin 8 shorting to pin 4. The Vin for this product is coming from a 53.5V output and it has an OVP of 60V, so we are sure the voltage is limited to 60V in the normal operation. LM5009 is rated Vin of 100V, may I know if this component LM5009 had some limitations that can cause short on Vin to Ground?
This is the output voltage of the power supply where we get the supply for this LM5009.
