Please see the observations below.

1) D104 should be SMDJ12A or equivalent.

2) Please place 1 nF ceramic capacitor across EV/UVLO pin and GND for better noise immunity.

3) SBR10U45SP5-13 or equivalent needs to be placed at the OUT pin to GND. TVS diode and Schottky diodes should be placed close to the device in layout.

4) Place a 22 pF capacitor across R45.

5) Place a 1 nF capacitor across IREF pin to GND.
