TPS54360: VIN to GND short failure

Hi experts,

Here is a good news we DIN TPS54360DDAR and now there is some failure happened to their boards. Over 20 pcs boards had both VIN-GND and SW-GND short failure in one day. I have test the waveforms on healthy boards and confirmed that the voltage of VIN-GND, SW-GND, BOOT-GND are not beyond the absolute operating conditions.

1. Could you please help to review the two parts schematics---Protection circuit and IC peripheral circuit? 

2. There is another notable point in failed boards is that the TVS are not short fail. The customer tried to recur the condition which cause such short failure. They applied even 6KV surge voltage via protection circuit but IC was still good. Could you please give kind opinions about how to recur the moment of failure?

    Here's a quick update.

    The schematic of the TPS54360  looks reasonable from my point of view. I'm not too familiar with the protection circuit but nothing strange pops up. A few things to note:

    1) Can you isolate the DC-DC converter portion and just check if you get the expected output voltage?

    2) Were you able to run a A-B-A test using a known good unit on a bad board?

    3) If TVS is damaged open, you may need to isolate that portion of the circuit and check voltage across TVS to see if rating is exceeded.
