    Please refer my comments below.

    • If it is not too late, please consider using LM5141. 
    • Traces are too thin. Please use wide copper traces. Please download https://www.ti.com/lit/ug/slvu984/slvu984.pdf and try to make the PCB layout similar with the example. 
    • It is recommended to separate AGND and PGND from the schematic level. 
    • Please compare your component selections with the Figure 12 and correct if any difference.
