There's nothing about the schematic that stands out wrong to me. Do you have any waveforms to share, or a bit more to describe in what manner of how the LM25184 failed?

I am loading on output side with 10mA of resistive load then the IC is failed.
Input side high current drawn and out voltage is not getting after loading.
IC over heated after removing the load and high current drawn on input side(24VDC).

It looks like you may have a short on your board.
For further information, can you provide the transformer that you were using?
Also, layout and waveforms will be very important in understanding what is going on since right now, otherwise it will be very difficult to find out the root cause.

Can you let me know what software you were using for PCB layout. It'll help with viewing the waveforms.
