BQ79616-Q1: VC0 pin damage

We seem to have a few instances where the lowest cell in a BQ79616 is reading low and causing cell imbalances.  Sometimes it is the bottom BQ chip in the stack, sometimes not.  I have gotten a hold of one board and the VC0 pin is sourcing current and the series R is dropping 30mV.  I measured a few other boards and "R72"  only drops about 0.4mV and is not sourcing.  A replacement board fixed the issue in the pack.  In this particular failed case it was the 3rd BQ in the stack.  I believe the BQ79616-Q1 is damaged in some way.  Has TI observed this issue with the VC0 pin sourcing current?
