If you're mainly looking for a starting point, the general equations for an Active Clamp Forward topology can be found here. I would also recommend downloading Power Stage Designer since it can simplify many calculations for your topology.

Section 4.3.1 in this app note seems relevant to what you are asking.
The main difference you will run into is that we will require some resistance in parallel with the clamp capacitor because our synchrnous rectifiers are off during soft start.
Thus you need to make sure to discharge the capacitor enough to avoid over voltage events.

I am looking for a schematic of an active clamp forward design, such as the push-pull schematic within the TI design file slvk099 file.
1) A primary side clamp FET connection circuit consisting of TPS7H5005 is required.
2) Which FET is the clamp FET composed of, N-ch FET or P-ch FET?

2) It can be either, it just changes where the FET connection is done
