Battery Charging and Power Path management

Dear Team,
We need a battery charging and power path management IC for one of our project. The requirements are listed below.
1. It can charge the Li-ion/Li-Polymer battery(3.7V Typical, 4.2V Max.) pack through USB.
2. Parameters(For example- Charge Current, Battery Voltage) can be programmable through I2C.
3. A constant 5V DC shall always be generated by the IC whether USB is available or not.
4. When USB is available, the USB supply shall be used for both supplying the current for output & charging the battery simultaneously.
5. The IC should support fast charging of the battery(3000mAh) as USB will be available for a short duration of 30 minutes max.
