LM5116: MOSFET damage during short circuit event

Part Number: LM5116

Hi.

We're experiencing a rare bug that leads to MOSFET damage (both HO and LO).

The preconditions are:

1) 60V input from lab PSU with 10A current limit.

2) A short circuit happens at the output connector on the end of the cable (~18 mOhm in total)

3) A short circuit happens in the steady mode (after Vout stabilizes)

The expected result is, of course, no damage during a short event. In most cases, it acts as expected. However, we have several reproduces with MOSFET+CS resistor (1206 thick film current sense) damage (possibly the CS resistor blows up after MOSFET).

Our schematic almost entirely utilizes the original Webench design (regarding semiconductor part numbers and values of the passive components).

Our schematic:

In summary, we need your assistance with finding out why the current protection doesn't work as expected (w/o damaging the half bridge)

We can provide the layout in private messages if it's necessary.
