DCP011512DB: DCP011512DBP Noise on the input

We're using this part and seeing a lot of noise generated by the input side of the device and being injected back into the supply.

Adding the recommend caps do nothing to help but adding an inductor / pi filter.

10uF - 10uH - 10uF

This cleans up the noise greatly but causes the DCP011512DBP to get very hot.

Any idea why it would get hot with an inductor on the input?
