TPS54521: TPS54521RHLR ends up short after power up or reset.

I would like to share with you our design for this DC-DC converter: TPS54521RHLR. We found out that this this chip goes short (output is short to ground) quiet often in the field due to a power cycle or reset. Could you please verify our circuit and see if there is anything to do to optimize our design and reduce this failure rate.

    Hi 

    I didn't see a higher risk of this sch that may damage IC. 

    Could you help capture the Vin, SW,Vout voltage when power cycle ? It would be good if you can capture the waveform when damage happening, for better analysis.
