Is there a short between Vout and GND?

I have a few suggestion/ questions:

decreasing C5 to 0.1uF

increasing the inductor to 15uH.

Do you see any changes to VREF if you increase or decrease the input voltage?

Have you tried replacing the IC?

What is the voltage on TERM_EN pin?
