LM5012-Q1: Short circuit

Hi,

I have designed a buck power supply based on LM5012QDDARQ1, wherein, I plan to convert an input dc of 40-80V (EV battery source) to a regulated 16V dc to power up my traction converter's gate supply. However, when I give the input to the board, I observe a failure of the said IC. Can you help me in identifying the fault here?
